What Is BOTOX® for Chronic Migraine?

BOTOX® (onabotulinumtoxinA) is an FDA-approved prescription treatment that helps prevent headaches in adults with chronic migraine. It is given on a regular schedule to reduce future headache days. You may still need rescue medication for headaches that occur between treatments.

Who May Benefit?

BOTOX® is approved for adults age 18 and older with chronic migraine who experience:

Headaches on 15 or more days each month.

Headaches lasting 4 hours or longer on those days.

Its safety and effectiveness for preventing episodic migraine—14 or fewer headache days per month—have not been established.

Our providers review your headache history, previous treatments, medications, and health conditions to determine whether BOTOX® is appropriate for you.

What to Expect During Treatment

Treatment involves small injections into specific muscles of the head and neck. The standard chronic migraine protocol includes 31 injection sites across seven muscle areas.

The injections usually take about 10 minutes, although your full appointment includes time to review symptoms and discuss your treatment plan. Most patients can go home shortly afterward.

Treatments are generally scheduled every 12 weeks.

When Will I Notice Improvement?

Some patients notice improvement within the first four weeks. Others need additional treatment cycles before the benefit becomes clear.

In clinical trials, patients receiving BOTOX® averaged approximately 8–9 fewer headache days per month at 24 weeks, compared with approximately 6–7 fewer days with placebo. Results vary, and treatment does not eliminate every headache.

Keeping a headache diary helps us track your response and guide ongoing care.

Possible Side Effects and Safety Information

Side effects may include neck pain, headache, injection-site discomfort, muscle weakness, or eyelid drooping.

BOTOX® carries a boxed warning about the possibility of toxin effects spreading beyond the injection area. Serious symptoms can occur hours to weeks after treatment. Seek immediate medical attention for difficulty swallowing, speaking, or breathing.

BOTOX® should not be given if you have an infection at the planned injection site or a known allergy to BOTOX® or another botulinum toxin product.

Before treatment, tell your provider about all medications, recent botulinum toxin injections, muscle or nerve disorders, swallowing or breathing problems, pregnancy plans, and breastfeeding.

Insurance and Treatment Costs

Coverage depends on your insurance plan and its treatment requirements. Prior authorization may be required, along with documentation of your headache frequency and previous medication trials.
